The CHRONICLE: Modernizing EHR for chronic disease surveillance

Michigan's public health has advanced with CHRONICLE, an EHR-based chronic disease surveillance system launched in 2019.
Leveraging the Michigan Health Information Network (MiHIN), CHRONICLE collects real-time admission, discharge, and transfer (ADT) data, initially focusing on stroke and hypertension.
By September 2022, it had captured 800 ADT messages daily, offering patient and trend analysis features.
CHRONICLE is a system that surpasses traditional surveillance limitations, provides detailed, timely data, and enables patient-level linkage across systems, ensuring its effectiveness in chronic disease surveillance.
Despite challenges like data quality and legal barriers, CHRONICLE is continuously enhanced.
This commitment to improvement instills confidence in its ability to include more chronic conditions and provide comprehensive disease surveillance.
CHRONICLE's future is promising and plans involve expanding data sources and integrating conditions like asthma and diabetes.
This expansion will support better Michigan disease management and health equity, fostering optimism about the system's potential impact.
