  • Michigan's public health has advanced with CHRONICLE, an EHR-based chronic disease surveillance system launched in 2019.

  • Leveraging the Michigan Health Information Network (MiHIN), CHRONICLE collects real-time admission, discharge, and transfer (ADT) data, initially focusing on stroke and hypertension.

  • By September 2022, it had captured 800 ADT messages daily, offering patient and trend analysis features.

  • CHRONICLE is a system that surpasses traditional surveillance limitations, provides detailed, timely data, and enables patient-level linkage across systems, ensuring its effectiveness in chronic disease surveillance.

  • Despite challenges like data quality and legal barriers, CHRONICLE is continuously enhanced.
